Consent Forms Required by Senate Bill 12
About
New legislation from the 89th Texas Legislative Session requires parents and guardians to provide annual consent for certain school-based services and activities.
Forms must be completed for each student by Friday, Sept. 12, 2025.
How to complete the consent forms in Google Forms
- Click here to start the Google Form.
- A parent or guardian must be the one filling out the form.
- Parent's Name, Phone Number, and Email Address are required.
- Student Name and Campus are required. Student ID# is helpful but not required.
- If you have additional children enrolled at an ECISD school, select "Yes" to add them. Select "No" once you have completed all of your children's information.
- Review each section:
- Health Services
- Counseling Services
- Indicate whether or not you consent in each category.
- Submit the form. East Central ISD will securely receive it. No further action is needed on your part.

Student Clubs/Organizations
Consent is required for a child to participate in school-sponsored clubs/organizations.
Please note: the consent form for student clubs/organizations is included when a parent/guardian acknowledges the Parent-Student Handbook.
- All extracurricular opportunities are reviewed and vetted to ensure compliance with state laws and district guidelines.
- Parents maintain the right to decide which activities are appropriate for their child.
