Background Check Requirements

For the safety of our students, all volunteers, clinical teachers, and university field residency students must complete and clear a criminal background check before beginning any on-campus activities.

Required forms (both are required):

  • Criminal History Release Form

  • DPS Release Form (Texas Department of Public Safety)
