REGISTRATION FOR 2009-2010 SCHOOL YEAR
Additional Immunization Requirements
All new and returning students must complete and return a registration packet.
2 Steps to Register for the 2009-2010 School Year
To the parents of East Central School District Students:
Each school in the District will be having registration for the
2009-2010 school year. Registration will be especially important for
the upcoming year.
Step 1
PICK UP OR PRINT REGISTRATION PACKETS. Packets will be available at
each campus starting July 1st or registration forms can be filled out
and printed from the ECISD website www.ecisd.net. starting July 15th.
Step 2
BRING ALL COMPLETED FORMS TO REGISTRATION WITH REQUIRED ID AND PROOF OF
RESIDENCY. (Home language surveys are to be completed only for students
who are new to the district.
**(Note the change in required proof of residency)**
Required during registration with all students will be a valid
Texas Driver's license or Texas ID of the enrolling parent/guardian and
one of the following additional requirements:
A current utility bill
will need to be provided for registration. The utility bill must be in
the parent/guardian's name and it must include their home address. Post
office boxes will not be acceptable. Included as an acceptable utility
bill are any of the following current bills with a computerized address
of the residence printed on the form:
* Electric Bill * Water Bill * Cable Bill * Home Telephone Bill
(OR)
A current rental/lease agreement will also be acceptable proof of residency if it contains all of the following information:
- Date of the agreement
- All family members residing in the dwelling
- Leasors' name and signature
- Lessees' name and signature
- Term of the lease/rental agreement
If the
parent/guardian cannot provide a current utility bill or current
lease/rental agreement, then a host form will need to be completed. All
host forms with specific requirements and instructions will be provided
at the ECISD Central Office in the Student Services Building.
Parent/Guardian registering students using a host form must be present during the registration process.
For students NEW to the District:
•
Copy of current proof of residency within East Central ISD (current
utility bill or current rental agreement with physical address listed
and move in date listed)
• If residing with someone, notarized HOST form along with current proof of residency described above from property owner
• Proof of identity:
- State certified birth certificate ( A public school is required to
identify a student by that student's legal surname as it appears on the
student's birth certificate)
- Social Security card (Student social security numbers are used for
purposes of the Public Education Information Management System;
however, if a student or his/her parent so chooses, the student may
request the district assign the student a state-approved alternative
student identification number.)
Immunization record
• Previous year's report card and/or school records
• Copy of court documents proving guardianship, if applicable
• Photo ID of enrolling parent/guardian
At registration, you will receive an application regarding
Free/Reduced meals; please call 648-7861, Child Nutrition Department,
if you need further assistance with this form.
Helpful Hints for Completing Registration Packet
Following instructions for the pages in the registration packet
will insure that you are not called to return to the campus if
information or copies are missing. This means that all forms should be
filled out completely!
Enrollment Form
- All blanks MUST be filled in
- Entry date will be 8/24/09 (first day of school)
- Previous school attended requires address and phone number
- Special programs information needed to continue service
- Legal documents needed to prove guardianship
- Sign and date
- Enrolling person must be parent or legal guardian
- Do not use abbreviations or ditto (") marks
Person Enrolling Student Form
- All students are required to have this form on file and it MUST be completed.
Permission to Pick Up Student Form
- All students are required to have this form on file and it MUST be completed.
Student Residency Questionnaire
- All students are required to have this form on file and it MUST be completed.
Family Survey
- All students are required to have this form on file and it MUST be completed.Home
Language Survey
- Must be completed by all students new to the district for 2009-2010.
Health Information Form
- Complete all information
- Sign and date
Acknowledgement Form
All students are required to have this form on file and it MUST be completed.
Ethnicity and Race Data Report Form
All students are required to have this form on file and it MUST be completed
